WHAT IS THE ENROLLMENT PROCESS?

• Required diagnosis of FIBROMYALGIA by Dr. Withers

  • Interested participants must have first New Patient appointment which can be billed through insurance or paid as self-pay.

• Submit an application form and enrollment fee

• Welcome package will be emailed – Complete documents and email or fax back to office prior to scheduling appointment

WILL I STILL HAVE VISITS WITH DR. WITHERS OR NURSE PRACTITIONER?

• Yes, the full features of the program include provider visits to ensure an integrative approach to managing your overall care.

HOW LONG DOES THE PROGRAM LAST?

• Boot Camp is 6 sessions

• Personalized Track extends ongoing care and is customized based on each participant’s need. There is no timeline for this phase of the program.

WHAT IF I CHOOSE NOT TO PARTICIPATE IN THIS PROGRAM?

• New patients seeking treatment for fibromyalgia will only be managed through the MyFibro program. 

• Any existing patients opting out of the MyFibro program will be referred back to their primary care doctor for further management.

WHAT If FIBROMYALGIA IS NOT MY ONLY DIAGNOSIS?

• If patients are being treated for another rheumatologic condition, he/she may continue regularly scheduled visits with our providers for care of any additional diagnosis. 

• Our providers will not discuss, advise, or prescribe medicines for fibromyalgia during visits or in phone messages after the indicated time if a patient opts out of MyFibro.

• Our Health Educator is professionally trained with knowledge and skills to promote wellness through behavior change in individuals and populations.
